Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs, developing personalized service plans, and connecting them with appropriate resources. This position requires strong interpersonal skills and a commitment to advocacy, ensuring that clients receive the necessary support to improve their well-being and achieve their goals.

Responsibilities

Conduct comprehensive assessments to identify client needs and strengths.
Develop individualized service plans in collaboration with clients, setting achievable goals.
Provide ongoing support and guidance through motivational interviewing techniques.
Monitor client progress and adjust service plans as necessary.
Facilitate access to community resources, including behavioral health services, child welfare programs, and workforce development opportunities.
Manage crises effectively, employing crisis management strategies to ensure client safety.
Supervise and mentor clients as they work towards their goals, providing encouragement and accountability.
Maintain accurate documentation of client interactions, progress notes, and case files in compliance with agency policies.
Collaborate with multidisciplinary teams to coordinate care and services for clients.
Stay informed about relevant laws, regulations, and best practices related to child welfare and social work.
Requirements

Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
Proven experience in case management or social services is highly desirable.
Strong knowledge of child welfare systems, behavioral health resources, and Section 8 housing assistance.
Excellent crisis management skills with the ability to remain calm under pressure.
Proficient in motivational interviewing techniques to foster client engagement.
Experience in mentoring or supervising individuals in a supportive capacity is a plus.
Familiarity with workforce development initiatives is beneficial.
Strong communication skills, both verbal and written, with an emphasis on empathy and active listening.
Ability to work independently as well as collaboratively within a team environment.
Commitment to maintaining confidentiality and ethical standards in all interactions.
